Local and District Presidents Community Service Award for Ninth Graders
RULES and INSTRUCTIONS
This award is given to a 9th grade student in recognition of his/her leadership, achievements, and service to the community in the field of music during the 7th, 8th, and 9th grades. Special emphasis is placed on service to the community which is the essence of the award.
The State winner and County winners are selected and announced no later than March 15th.
Each county winner is presented a County Winner Certificate by FSMTA, additional awards may be given by the local MTA. The State winner will receive a plaque and a check for $250.00.

Eligibility:
- Applicant must be finishing the 9th grade
- Applicant must be a resident in the State of Florida
- Applicant must be studying with an FSMTA-member teacher.

- Application for Local Presidents FSMTA Community Service Award. Download and complete this application form including the Musical Community Service Activities, Musical Service Activities within School Activities, School Information, Short Biography, and Applicant Statement. Send as an email attachment to [email protected].
- School Administrator/Teacher Letter of Recommendation. Download and have a letter of recommendation from your Principal, Vice Principal, Music, Band, or Choral Teacher/Director completed. Send as an email attachment to [email protected].
- FSMTA Teacher Letter of Recommendation. Download and have a letter of recommendation from your music teacher completed. Send as an email attachment to [email protected].
- A recent photo of Applicant. Please send your photo as an email attachment to [email protected].
- IMPORTANT: Application fee of $40 must be paid using the PayPal button below by the local MTA in which the Teacher is a member. Applicant’s FSMTA teacher must request the local treasurer to make the PayPal payment below. The name of the applicant must be identified in the PayPal payment screen. The fee is due January 17th. If payment is not received by the deadline, the application will not be processed. If the applicant lives in a county that is not part of the Local MTA in which the Teacher is a member, it is the responsibility of the teacher to pay the $40 fee.
Procedures:
- If there is more than one entry from any county then the District Community Service Chairperson will be notified and all information sent to District Chairperson for a selection of the county winner.
- If there is more than one county within a District then there may be winners in each county. The winner is selected and information given to State Chairperson by February 15th.
- The selection of the State winner is made by the FSMTA Community Service Committee.
- The State winner will be announced no later than March 15th and will receive a plaque, certificate, and a check for $250.00
- Each county winner is presented a County Winner Certificate. Local MTA may elect to award a County Winner plaque.
